To understand why burning feet has so many causes, you have to understand which nerves are doing the talking. Your peripheral nerves carry different fiber types. Large, heavily myelinated fibers handle vibration, position sense, and strength. Small, thinly myelinated and unmyelinated fibers — the small fibers — carry pain and temperature. Burning is the signature complaint of the small fibers. When they are irritated, damaged, or firing abnormally, the brain interprets that traffic as heat and burning, often most intensely in the feet because that is where the longest nerves in your body end.

This is the one everyone knows, and for good reason. Diabetes is the most common cause of peripheral neuropathy worldwide. Burning feet can be a warning sign of diabetic nerve damage. Roughly half of people with diabetes develop distal symmetric polyneuropathy over their lifetime, and about 15 to 20 percent experience the painful, burning form (large cohort data; Diabetes Care, 2024). The damage follows a classic stocking-glove pattern: symmetric, starting in both feet, slowly moving upward before the hands are ever involved.

What most people miss is that you do not need a diabetes diagnosis for high glucose to be damaging nerves. Prediabetes and impaired glucose tolerance are strongly associated with small fiber neuropathy, and elevated blood sugar can damage nerves years before diabetes is formally diagnosed or before blood sugar crosses the diabetic threshold. Nutritional causes: B12 deficiency, B6 toxicity, and alcohol

Nerves are metabolically demanding tissue, and a handful of nutrient problems show up as burning feet. Vitamin B12 deficiency is the classic one. It is common, easy to test for, and often missed — particularly in older adults, people on long-term acid-reducing medication or metformin, those following plant-based diets, and people after weight loss surgery, especially gastric bypass.

Here is the counter-intuitive part that surprises patients: too much vitamin B6 can cause the very neuropathy people take it to prevent. This was classically reported with very high doses (hundreds to over a thousand milligrams per day from stacked supplements), but newer safety alerts describe sensory neuropathy at chronic intakes above 100 mg per day. Medication-induced nerve injury

Some of the most effective drugs in medicine are toxic to peripheral nerves. Chemotherapy drugs — platinum-based agents in particular — are the best-known example, and chemotherapy-induced peripheral neuropathy frequently presents as burning hands and feet. Exposure to toxic chemicals can also injure peripheral nerves. Certain antibiotics, antivirals, and other medications can do the same. The clinical clue here is timing: if the burning began within weeks to months of starting a new medication, that relationship deserves a hard look. This is never a reason to stop a prescribed drug on your own — it is a reason to have the conversation with the physician who prescribed it.

Structural and compression causes: tarsal tunnel syndrome and where the surgery lives

This is the category most often mislabeled as generic "neuropathy," and it is the one I care about most, because some of these are correctable. When a nerve is physically pinched, it burns — but the pattern is different from a metabolic neuropathy.

Tarsal tunnel syndrome is the foot's version of carpal tunnel. The tibial nerve gets compressed as it passes behind the inner ankle, sometimes by tight surrounding tissues or bone spurs, producing burning and tingling on the sole that is often worse with activity and standing, and frequently affects one foot more than the other. Lumbar radiculopathy — an irritated nerve root in the lower back — can send burning down the leg into the foot, usually with a stripe-like distribution and often accompanied by back pain. Other entrapment neuropathies follow the anatomy of the specific nerve involved. The reason this matters: a compressed nerve has a physical problem with a physical solution. Nerve decompression surgery can relieve the pressure, and outcomes are often better with earlier release, before chronic compression causes permanent axonal loss. A truly symmetric, both-feet-equally metabolic neuropathy is not a surgical problem. A one-sided, activity-related, positionally triggered burn may well be.

Vascular, skin, and the great mimic: erythromelalgia

Not every burning foot is a nerve problem. Poor circulation can cause burning, aching, and color changes, particularly with walking, and changes in blood flow or problems in the blood vessels can contribute to those symptoms. Skin conditions, including contact dermatitis, and fungal infections such as athlete's foot can produce burning and itching that has nothing to do with the nerves at all — a genuinely good-news scenario, because it is easily treated, though fungus tends to thrive in damp shoes and on locker room floors.

Then there is erythromelalgia, a rare neurovascular condition worth knowing about because its pattern is so distinctive. The feet become intensely red, hot, and burning in episodes, and — unlike almost every other cause — the symptoms are triggered by warmth and dramatically relieved by cooling (Mayo Clinic Proceedings, 2022). Patients describe plunging their feet into cold water for relief. One caution: some people over-cool their feet with ice or prolonged immersion and damage the skin, so if you recognize this pattern, talk with your physician about safer cooling strategies. If your burning comes in fiery, red-hot attacks that improve when you cool the feet, that specific story is worth bringing to your physician by name.

How to Tell the Difference: Reading the Clues

You cannot diagnose yourself from a symptom list, and you should not try. But you can notice the details that help your physician narrow the field quickly. When I evaluate a patient with burning feet, I am reading a pattern, not a single symptom. A few of the details that carry the most weight:

Distribution and symmetry. Both feet equally suggests metabolic or systemic. One foot, or one part of one foot, suggests a local or compressive cause. A stripe running down the leg suggests a nerve root in the back.

Burning versus numbness. Small-fiber problems often cause a burning sensation or other nerve pain. Large-fiber problems bring numbness, imbalance, and loss of position sense. Many people have both, and the mix is informative.

Burning that comes in discrete red-hot episodes points toward erythromelalgia. Burning that tracks a new medication points toward that drug.

What makes it better or worse. Relieved by cooling is a specific clue. Worse with walking or standing raises circulation or a compressed nerve. Worse after alcohol raises alcohol-related injury.

Associated symptoms. Weakness, swelling, skin color or temperature changes, balance problems, back pain, or symptoms in the hands and lower legs — each of these steers the differential. If symptoms spread beyond the feet, that can suggest broader nerve involvement.

How Physicians Evaluate Burning Feet

A proper workup is not complicated, but it is systematic, and the goal is an accurate diagnosis, not just putting a label on symptoms. It starts with a thorough history and physical exam — including simple bedside tests like monofilament touch and vibration sensation, plus an exam of the feet and lower legs, to reveal which fibers are affected. From there, the workup is targeted:

Blood tests screen for the common and treatable causes: glucose and hemoglobin A1c (and often a glucose tolerance test to catch prediabetes), B12, thyroid function, kidney function, and markers of autoimmune or inflammatory disease. Nerve conduction studies and EMG assess nerve function in the large fibers and can identify compression or more widespread neuropathy — but remember they can be normal in pure small-fiber disease. When small fiber neuropathy is suspected despite normal EMG, a skin biopsy measuring intraepidermal nerve fiber density is the test that actually confirms it (Cleveland Clinic Journal of Medicine), and unclear cases may still need further testing. Imaging comes in when a structural cause — a compressed nerve, a lumbar nerve root, or a related issue closer to the spinal cord — is on the table.

The point of all this is not to run every test on every patient. It is to let the clues from your history direct the workup so the right answer surfaces efficiently.

Why the Cause Has to Come First

Here is the through-line of everything above: treatment depends entirely on the cause, so good care should provide symptom relief while also addressing the underlying problem. B12 deficiency is corrected with B12. A compressed nerve may need decompression. Prediabetic nerve injury responds to glycemic control and the metabolic work that supports it, which can also reduce symptoms in some patients. Erythromelalgia is managed in a completely different way. A fungal infection needs an antifungal, not a nerve drug. Prescribing the same "neuropathy" protocol to all of them guarantees that most patients get a treatment aimed at the wrong target.
